Can a Person Who Takes ARVs Infect Others? The U=U Guide

If you have recently received a positive HIV diagnosis, or if you are supporting someone who has, your mind is likely racing with questions. One of the most common questions we hear at Agatha’s Space is: “Can a person who takes ARVs (antiretrovirals) infect others?”

The short answer, backed by years of global medical research, is no. When treatment is taken correctly and the virus becomes undetectable, the risk of transmission is zero. This is known as U=U (Undetectable = Untransmittable).

In this guide, we will break down the science, the statistics, and what this means for your life and relationships.

What Does U=U Actually Mean?

To understand how someone on treatment cannot pass on the virus, we use the Photocopier Analogy.

Think of HIV as a photocopier inside the body that is constantly making copies of itself. Without medication, those copies fill up the blood. Antiretroviral therapy (ARVs) acts as the "OFF" switch for that machine.

Once the machine is switched off, the number of copies in the blood drops so low that standard lab tests cannot even see them. This is called having an Undetectable Viral Load

Medical experts at the British HIV Association (BHIVA) confirm that if your viral load stays undetectable for at least six months, there is zero risk of passing HIV on to sexual partners.

The Statistics: Why We are Confident

The "Zero Risk" message isn't just a hopeful thought; it is a scientific certainty proven by massive global studies.

  • The PARTNER Studies: Researchers monitored over 1,000 couples where one partner was HIV positive (and undetectable) and the other was negative. After nearly 77,000 acts of condomless sex, there were zero cases of HIV transmission. Read the full study results here.

  • UK Progress: According to the UK Health Security Agency, the UK is a global leader in U=U. Approximately 98% of people diagnosed with HIV in the UK are on treatment, and of those, 97% are virally suppressed (undetectable).

This means that almost everyone in the UK who is engaged in regular HIV care is unable to pass the virus on.

Real Voices: Life Beyond the Diagnosis

For many of our sisters, the U=U message is the key that unlocks a life free from shame.

"When I first found out, I thought I was a danger to my family. I stopped hugging my children. But when the doctor explained U=U, it was like a weight lifted. I’m undetectable now. I have a partner who is negative, and we don't live in fear. The science gave me my dignity back." > - Sarah, Peer Mentor at Agatha's Space

Knowing that you cannot infect others allows you to breathe, to date, and to live without the "stopwatch" of fear running in your mind.

How Do I Get Undetectable?

The journey from "Detectable" to "Undetectable" is a process, not a miracle. It requires:

  1. Testing: Knowing your status so you can start the journey.

  2. Consistency: Taking your ARVs every day as prescribed.

  3. Monitoring: Regular blood tests to check your viral load.
